Checks for understanding throughout the lesson to monitor student learning.
Everyday during math I have the students independently work on what are called "Problem Sets" after I finish giving my math lesson. The students essentially are doing the same work that I did during direct instruction, but with different numbers on their own. We then check our answers together as a class towards the end of our lesson.

During guided reading, I group my students into similar reading levels. While reading the book, I constantly assess the student(s) throughout their reading lesson to check their understanding. I utilize my guided reading sheet to scaffold the students reading. I will ask them comprehension questions as well as questions about what they have read during the reading lesson to check for understanding.
Here is an example of a lower level group's book in my fourth grade class. As we read this book I would guide them by asking comprehension questions, vocabulary questions and more.
